Maryland Tax Forms:

Free Tax Help:

Lifestyles of Maryland Foundation, Inc. will provide free IRS-certified tax return preparation assistance at the libraries for households with income of $52,000 or less. Appointment is necessary.  Schedule an appointment by calling 1-866-293-0623. 

What to bring: 

  • Photo identification for tax filers
  • Social Security Card and/or Individual Taxpayer Identification Number (ITIN) cards for everyone on the return
  • W-2, 1099 and any other income forms
  • Documentation for deductions & tax credits
  • Proof of bank account, routing numbers and account numbers fro Direct Deposit (if applicable)
  • A copy of last year’s federal and state returns (if applicable)
